REGISTRATION

Registration can be completed online at any time or in person during regular business hours, 7:30am - 4:00pm.

You will need:

• Photo ID

• Social Security Card (CNA Class only)

• Payment

We accept cash, check, money order and all major credit cards.

All transactions for CNA classes have a non-refundable background check fee of $75, this will allow QHO to run the background check first to qualify you for the program. The background form must be completed and returned to QHO within three days. QHO will notify you once your background check has been completed and then will confirm your placement in class. If your background info has not been received within three days you will then go into a holding spot where your registration can not be guaranteed. It is your responsibility to notify the office.

As soon as you have been accepted into the program you may pay the remainder of the tuition.
